When you are perturbed, it means you that you are very worried about something that has happened or will happen.You are agitated or in a confused state of mind and you actually feel very upset about something.So when you are anxious due to something that has happened, you are perturbed.
As the word perturb shows the action of disturbing or being upset, it is a verb.
For example, your friend's ill health perturbs you, that means you are very worried about it and the situation she is in upsets you very much.
